I have struggled with this condition since I was a child, at times very severely. Usually I would pick my cuticles (to the point of terrible pain and embarrassing wounds/bleeding), but a couple of times I picked the heel of my foot deeply to the point of very severe pain and limping, and even then couldn't stop picking it.

I know this solution isn't exactly simple or easy but I just want to point out that it is the only thing that has worked for me. Sometimes in life we become so accustomed to chronic stress that we don't even realize it's there which can make it harder to solve. But if you can figure out if there are ongoing financial or family/relationship issues in your life for example, and make changes, it can solve this devastating issue. I'm not going to say it's 100% gone, however it no longer has negative effects on my life. I may do it mildly here and there, but it's almost nothing at all compared to before. Meditation, yoga, etc, also help a lot. Anything that calms stress.

The other thing that helped is using a nail clipper and cuticle scissors instead of picking. It still does damage, but much less than picking and tearing at the skin. And on my feet I am diligent about using a pumice stone in the shower every time, and a heavy duty foot grater, so that there is never anything to pick at.
